Send us your intake form- filled out with as much detail as possible. This helps us to serve you more efficiently.  After we receive your form, we will call you to set up an appointment either at a central location, a house call, or a in-house visit.

If for some reason you cannot fill out the intake form, call us- we will figure out another option for you.
